Abstract

Uterine manipulator helps the surgeon to hold uterus, in a desired position,for cutting it from its attachments. It also keeps important structures away from the energy sources and protect them. The advantages of manipulator are Good visualisation Keep Bladder, bowel and ureter away from operating field. There are various types of manipulators available. They move the uterus, back and forth, left and right. The attached colpotomiser, identifies vagina, and helps keep urinary bladder away. Manipulators are used via vaginal route. Some use myoma screw to manipulate uterus. This method, occupies one port. Myoma screw is very helpful in big sized uterus. In certain conditions, where manipulator can't be used vaginally are Very big uterus Space occupying fibroids in cavity of uterus, Elongated cervix Vaginal stenosis Cx not visualised due to adhesions In cervical hypertrophy, colpotomiser is difficult to introduce.

In such cases, myoma screw is helpful. In myoma screw manipulation, orientation is difficult, especially when uterine arteries are done. With this introduction, we can say that there is no single method of uterine manipulation is superior than others. A new manipulator is developed to increase utility of manipulator. The advantages of new manipulator are It can fit when uterine cavity is obliterated by fibroid. Conventional colpotomiser can be used. Easy to use Sturdy Autoclavable Can rotate uterus along its long axis. This helps bring the pedicles anteriorly, keeping away from vital structures. Posterior colpotomy is easier and more safe. Needs less dilatation. It is assistant friendly. This consists of four parts. Two arms, one sheath, and a handle for giving position. First, two arms are introduced in uterus. Then, sheath is introduced. This sheath is loaded by colpotomiser. Then handle is attached. All parts are autoclavable. The two arms, when locked, look like Copper T. The arms can get accommodated in abnormal uterine shape also.
